Why is there no "x_ship_to_phone" field on the SIM payment form?
Am I the only one that may have someone using a personal card and shipping to their work address?

There isn't a ship to phone number field supported in Authorize.Net. You can use merchant defined fields for the purposes of submitting data not allocatted to supported fields if you'd like.
