CCV Best Practices

Sorry if this seems repetitive but I haven't found an answer that addresses these issues yet.

 

We accept MC, Visa, Discover, and AmEx. I'd like to require CCV for all purchases but I have two concerns: 

 

First, does every user of every one of these cards everywhere in the world have a CCV code? If not, I can't require it, obviously, without losing sales.

 

Second, how do I handle people like me whose cards are so worn that the CCV code is no longer visible? If I give people the option to omit the CCV, then I have effectively removed the requirement for everyone (since an empty CCV value is interpreted as "do not test CCV" with the DPM integration method I'm using).
